Rifaximin is a nonsystemic antibiotic, meaning it targets the gastrointestinal tract specifically. It inhibits RNA polymerase, which results in the inhibition of bacterial protein synthesis and killing of the microorganisms. This action is what makes it a broad-spectrum antibiotic, active against various gram-positive and gram-negative bacteria, including anaerobic bacteria. When traveler's diarrhea occurs, rifaximin is administered to inhibit the mobilization of bile acids from the liver, minimizing the growth of bacteria in the gastrointestinal tract known to cause this condition. It is also believed to have a direct antimicrobial effect on the bacteria itself. In terms of hepatic encephalopathy, rifaximin can help reduce the risk of breakthrough infections by reducing the luminal bacterial load, altering the microbiome, and decreasing production of toxic metabolites by intestinal bacteria.
